DO you receive or have you claimed Disability Living Allowance (DLA) or Attendance Allowance? How easy to complete were the 20 plus page forms? Are you happy with the outcome of your claim?
Can you spare a few minutes to help with a short survey? If so, Disability Advice and Information Services (DAIS) Limited need your help.
Government figures show that not as many people claim DLA and Attendance Allowance as expected. This may be because the complicated, lengthy claim forms confuse many people or perhaps they are scared to claim benefits for fear they will be labelled as scroungers.
Our company would like to test if this is the case and needs to hear from people who have experience of claiming DLA or Attendance Allowance, particularly if their claim was refused.
If you can spare a few minutes to help us with our research, please ring us on 01257 424000 and we will send you a short questionnaire to complete. Your answers can make a difference and your personal details will not be revealed to anyone.
