York LINk (Local Involvement Network) helps local people and organisations have their say on health and social care services.

One of the issues we have been looking at during the past year is end-of-life care services in York. People approaching the end of their life may be cared for at home, in a care home, in hospital or in a hospice.

Regardless of where they are and who is caring for them, everyone has the right to expect high-quality care at the end of their life.

This is also an important issue for families and carers, both before and after their loved one dies.

End-of-life care services have undergone a number of changes recently, and we would like to hear from anyone in York who has experienced these services during the past six months.

Please let us know what you think about the care and support given to you and your loved one – what worked well, and what didn’t work so well.

York LINk will shortly be publishing a report on end-of-life care services which will be sent to organisations who arrange the services in York. All information given to the LINk will be included in the report anonymously.

Personal details will not be passed on and will be treated confidentially. Your feedback will help to make a difference for families in the future.
