CHESWORTH Janet (Jan) Passed away peacefully on April 21st, 2015, in York Hospital, after a short illness, aged 61 years. Beloved wife of Jim, much loved mum of Tessa and Allan, dear daughter of Joyce and sister of Sharon and John, also an adored grandma of Lyla. Funeral service at York Crematorium on Thursday, May 7th, at 3.40 pm. Family flowers only please, donations in lieu if desired to Martin House Children's Hospice.
